Recent research has noted the importance of inflammation in diabetes, with a central role for IL-1 beta. Thus therapies that target IL-1-beta could represent a promising new treatment strategy. This report reviews the evidence and rationale for use of anti-IL1 therapies in both type 1 and type 2 diabetes.

XOMA's anti-IL1 beta antibody is in phase II trials and data are expected by year-end. Several other IL-1 inhibitors are also in clinical studies for type 2 and/or type 1 diabetes, including Regeneron's rilonacept, Novartis' canakinumab and Biovitrum's anakinra. While there is good rationale for testing these drugs in diabetes, there are lingering questions about safety and efficacy, which are reviewed herein. In addition, the market opportunity for these drugs may not be as good as it first appears. Data are limited in type 1 diabetes and competition is intense in type 2 diabetes.
